Frequently asked questions · Health card

The health card does not arrive or has wrong data: what should I do?

Direct answer

First distinguish four situations: the card was never issued, it was mailed but not delivered, personal details are incorrect, or a replacement is needed. The local health authority (ASL) checks National Health Service (SSN) registration and data transmission to Sistema TS; the municipality or Italian Revenue Agency corrects registry details as appropriate.

Who this applies to

People with an Italian tax code who are registered with the SSN, including eligible foreign residents. Someone not registered with the SSN may receive a tax-code card rather than a health card.

Practical steps

  1. If no card was issued, ask the ASL patient-registration desk to check your SSN registration and whether it was transmitted to Sistema TS, showing your identity document and tax code
  2. if it was issued but not delivered, check the residence address held by your municipality, since delivery follows the address in the tax registry
  3. if your name or date of birth is wrong, residents can request a correction through their municipality or via ANPR under 'I tuoi dati' > 'Rettifica dati' using SPID/CIE/CNS; non-residents should contact an Agenzia delle Entrate office with valid ID
  4. after records are corrected, request a replacement if needed using the appropriate Revenue Agency or ASL service; for a chip-enabled TS/CNS, the online replacement service is inside the Revenue Agency's authenticated area. Keep evidence of requests.

Documents to keep

Identity document, tax code or incorrect card, documents confirming correct personal details, and any SSN registration evidence or request receipt. Attach supporting evidence when ANPR asks for it.

Verifiable timing and costs

Standard issuance of the health card is free. There is no single guaranteed national delivery deadline for every case; establish the record and mailing status with the responsible agency before requesting another issuance.

Exceptions and special cases

If the tax code is missing or the ASL has not transmitted the registration, those records must be corrected first. The online replacement for a chip-enabled TS/CNS is in the Revenue Agency's authenticated area, not the public service for different tax-code card cases. ANPR corrects resident registry data, not the health card directly.

If the procedure stalls

If the ASL confirms registration but no card is issued, request a check of data transmission to Sistema TS and keep the reply. If the address or personal data are incorrect, document the correction before requesting a replacement. For healthcare needs, ask the ASL to verify your coverage; absence of the physical card does not automatically terminate registration.

Practical example

Example: Samira renews her residence permit but receives no health card. At the ASL she discovers her SSN registration was not updated; she provides renewal documents and keeps confirmation that her record was corrected. If the card had already been mailed, she would check the registered address first.
